Authorized Signers Information
Each company will receive customized, bar-coded applications for all ACTIVE employees. Applications for employees whose badges expire during the August-September rollout period will be stamped RENEWAL.
APPLICATIONS
SECTION 1 – Badge Holder Information
Your employee's full name, DOB, last 4 digits of their SSN, job title and your company name will be pre-printed on the form. Only minimal information will need to be filled in by your employees. Refer to the ‘BADGE HOLDER' Section. If the employee has moved since the current badge was issued, they must write in new information. Otherwise, they can leave the address section blank.
SECTION 2 – Primary Employer
Company Authorized Signers must complete Section 2 certifying that the employee requires a NEW SERIES badge.
SECTION 3 – Additional Employer
Employees who work for more than one airport company, must obtain the authorized signature from their secondary employer. NEW SERIES badges will display both companies' names on the badge. If we do not have an authorized signature for the Additional Employer at the time the NEW SERIES badge is issued, the employee will not be authorized to work for the 2nd company. The employee will need to come to the Badging Office with a Badge Renewal form and a $15 badge fee will be impose.
ID REQUIREMENTS
Employees must bring their MSP Security Badge and one government-issued photo ID. (Non-citizens must bring their current INS primary documents.) If your employee has changed their name, they must bring a court document (marriage certificate, etc.) showing the official name change.
COST
NEW SERIES badges (non-expiring) will be issued free of charge during the August-September rollout period. Badges up for renewal during this period will be subject to the new $15 badge fee.
NEW SERIES BADGE LOCATION
NEW SERIES badges will be issued on Level 6 of the Hub Building between Red & Blue parking ramps.
QUESTIONS
Q: What about employees whose badges will expire during the August-September rollout?
A: If your employee's application is stamped RENEWAL, send them to the Badging Office.
Q: What if I'm missing an application for one of my employees?
A : Please call 612-726-5427 or 612-467-0519. There are several reasons why this might have occurred and will be handled on a case by case basis.
Q: My employee lost his form. What should he do?
A: The authorized signer should contact 612-726-5427 or 612-467-0519 for replacement form.
Q: Several employees will be on leave during the rollout. What should they do?
A: Employees should bring the NEW SERIES badge application to the Badging Office and a $15 badge fee will apply. All old series badges will be disabled on October 2.
Q: I have applications for people who have been terminated. What should I do with them?
A: Call or e-mail the Badging Office immediately to deactivate their badges. Return unused forms to the Badging Office.
